The visitor form is for the visiting student to fill out; bring it to the visit date and turn it in at the office. (The host student does not fill out anything on this form.) |

Site Visit Form - Host |
The host form is for the Oakland Tech host student to fill out. The host student must have his or her teachers sign it, before the Shadow Day. Please give the teachers some reasonable amount of warning (in case there's a test or something). A couple of days to a week is fine. You do not have to get the principal's office to sign before the shadow date, but you can if you want. |
